JNC-7 Guidelines for Treatment of High Blood Pressure



The National Heart Lung and Blood Institute(NHLBI) publishes clinical guidelines for the treatment of hypertension. The Seventh Report of the Joint National Committee on the Prevention, Detection, Evaluation and Treatment of High Blood Pressure, otherwise known as JNC-7, was published in 2003 and is currently in revision for JNC-8 due out in March 2010 for comment and in final form in summer of 2010.

A JNC-7 Express Guide has been written for primary care.

The Electronic Preventive Services Selector (ePSS) is an application designed to help primary care clinicians identify the screening, counseling, and preventive medication services that are appropriate for their patients.

The ePSS is available both as a web application and a mobile application. The ePSS information is based on the current recommendations of the U.S. Preventive Services Task Force and can be searched by specific patient characteristics, such as age, sex, and selected behavioral risk factors.

How do I find an article? Let Us Count the Ways




Where do I find articles? In 1959, Librarian Herman H. Henkle's print journal collection, might have been your source! But in 2009, you have more options.

There are still a few print journals left on the shelves at Montgomery Hospital's Library, but most of your articles will come from three sources:


1. Medline is the largest database of article information. The National Library of Medicine(NLM) funded by the U.S. government, employs a large staff of librarians to summarize medical articles, and assign subject headings/tags to each one. The data is freely accessible, and many companies repackage it with different interfaces.

PubMed Medline: This interface is produced by the NLM, and is available from any computer for free. If you want free full text though, only 10% of the articles have a link to the full text online from the publisher. To restrict to full text articles, click the limits button, and choose the "free full text" option.

NOTE: Due to the closure of the library, no interlibrary loan is available.

2. Interlibrary Loan, otherwise known as "ILL," is your key to those several thousand other journals that aren't free in Medline.

3 NEJM: The current print issues are at the library. You can also get to the NEJM online versions from any networked pc in the hospital. The publisher will recognize your IP address, and log you in, if you are at the hospital.

4. Google: Sometimes, if you are fortunate, a publisher will post an article in free full text to the web, perhaps because of the public health implications, or just because they feel like it.

New Clinical Practice Guidelines for Clostridium difficile Infection in Adults


The Society for Healthcare Epidemiology of America is offering access to the new
Clinical Practice Guidelines for Clostridium difficile Infection in Adults
SHEA/IDSA
ICHE May 2010

This guideline is designed to improve the diagnosis and management of Clostridium difficile infection (CDI) in adult patients. A case of CDI is defined by the presence of symptoms (usually diarrhea) and either a stool test positive for C. difficile toxins or toxigenic C. difficile, or colonoscopic or histopathologic findings revealing pseudomembranous colitis. In addition to diagnosis and management, recommended methods of infection control and environmental management of the pathogen are presented. The recommendations are based on the best available evidence and practices, as determined by a joint Expert Panel appointed by SHEA and the Infectious Diseases Society of America (IDSA) (the SHEA‐IDSA Expert Panel). The use of these guidelines can be impacted by the size of the institution and the resources, both financial and laboratory, available in the particular clinical setting.

UpToDate Online: Expert Opinion Clinical Reference


The Library subscribes to UpToDate Online. It is available from any networked pc at the hospital--once at the site, just click on "Accept" for the license agreement. Unfortunately, there is no remote access, pda access, or cme available through institutional subscriptions.

UpToDate is a practical clinical reference designed to provide quick access to synthesized medical information in an easy-to-use format. It is comprised of thousands of original topic reviews written by a recognized faculty of experts who each address a specific clinical issue and provide detailed recommendations.

MRSA found in 4% of Healthcare Workers

According to Medscape Medical News, approximately 4% of healthcare workers tested positive for methicillin-resistant Staphylococcus aureus (MRSA) in a large screening effort, a new analysis reports.

Carlos Guerra, MD, from the Brody School of Medicine at East Carolina University in Greenville, North Carolina, presented the findings here at the Fifth Decennial International Conference on Healthcare-Associated Infections 2010.

According to Kerri A. Thom, MD, MS, assistant professor of epidemiology and preventive medicine from the University of Maryland in Baltimore, other studies have shown a similar rate of MRSA carriage in healthcare workers.

Dr. Thom emphasized that all healthcare workers should practice Standard Precautions when caring for patients. "By following these guidelines, healthcare workers are protecting both themselves and their patients from the transmission of potential pathogens."
